To submit an application or request through USOSweb, please log in and navigate to the “Student Section.”
To submit your sick leave confirmation to the Graduate Dean’s Office, please ensure that it is prepared by a medical doctor and attached to your application for rescheduling your exam date.
The confirmation of sick leave must also be emailed to the Student’s Support Person within three days of issuance from the SGH mailbox.
Applications submitted in paper format must include the following components:
- data of the person: student’s number, semester of the studies the student is in, correspondence address, telephone number,
- indication of the demand,
- signature of the person applying.

IStudents must submit documentation confirming the reason for absence (e.g., a medical certificate covering the exam date or other circumstances preventing attendance) by email from an sgh.waw.pl domain to the designated Student Support Person within three days of the occurrence.

Dropping Courses after the Deregistration Window
Incoming exchange students may request to drop courses after the deregistration window has closed, within three (3) weeks from the start of the semester.
Requests must be submitted via the General Application form in USOSweb and must include:- a detailed justification;
- if included in the Learning Agreement (LA): an opinion from the sending institution’s coordinator (signed letter, official email, or approved changes to the LA).
- in rare cases that exceed the regular Changes to LA period - consent of the course instructor(s);
Important: Missing any required attachment may result in rejection due to formal deficiencies.
Before submitting a request, please ensure there are no timetable clashes and that ECTS requirements are met.
All attachments must be submitted in PDF format. Documents issued in languages other than English or Polish must include a brief English summary.Adding Courses after the Registration with the Teacher’s permission
Incoming exchange students may request to register for courses after the registration with the teacher’s permission in USOSweb has closed and no later than three (3) weeks from the start of the semester.

Please note that the answer to the application submitted to USOS will be sent via USOS.
Application for changing surname or other personal data
Students are required to inform the school of any changes to their personal information that are relevant to their studies. This includes changes to their name, surname, permanent address, and correspondence address. If a student does not notify the school of a new permanent or correspondence address, any letters or decisions sent to the previous address listed with the school in Poland will be considered effectively delivered and will have legal consequences.
To report any changes to personal information, students must submit a statement in person to their Student Support Person during office hours. They should present the original document that verifies the changes. If a student changes their phone number or correspondence address, they can confirm this change via mail or by sending a scanned document from their SGH email account.
